چکیده

Introduction: Chronic osteomyelitis and infected non-union are relatively rare conditions in pediatric patients and are more frequently observed in the developing countries. Although relatively rare, they are challenging medical and surgical issues. The current study aimed to present a novel surgical technique used to manage three patients with chronic osteomyelitis of long bones. Case Presentation: Three skeletally immature patients with chronic osteomyelitis and infected non-union of the long bones were treated surgically from 2010 to 2013 through infected site debridement of bone and soft tissues, excision of sequestrum, irrigation and antibiotic-laden cement spacer, to fill the bone defect zone, were performed in all patients. All patients underwent magnetic resonance imaging (MRI), computed tomography (CT) scan and laboratory evaluation prior to surgery. Antibiotic regimen started empirically and was adjusted according to the culture and sensitivity results. Once inflammatory markers normalized, all patients were re-operated for cement removal, bone substitute graft and concomitant osteosynthesis of the affected bone. The three patients aged 14 (two patients) and 10 years (one patient) at the time of injury. All patients had at least two years follow-up (range 2-5). Clinical and laboratory evaluation had been normalized, bone healed and all patients had returned to daily life and sport activities. Conclusions: Surgical debridement is the standard approach to chronic osteomyelitis. Since antibiotic therapy plays an adjunctive role, it is recommended to use antibiotic-laden cement to penetrate local infection. The cement also induces membrane formation that aids bone reconstruction.

Isolated Chronic Osteomyelitis of Fibula in a Child: A Case Report

Background The most common sites affected in pediatric osteomyelitis are long bones of lower extremities such as femur and tibia but isolated fibular osteomyelitis has rarely been reported in children. Here, we present a case of isolated chronic osteomyelitis of fibula in a 2.5-year-old girl. To our knowledge, this is the youngest patient repor...

Gentamycin-impregnated calcium phosphate cement for calcaneal osteomyelitis: a case report.

We report a case of chronic calcaneal osteomyelitis in a diabetic patient who was successfully treated with radical debridement and gentamycin-impregnated calcium phosphate cement. At 1.5-year follow-up, the patient could walk without any assistance. Calcium phosphate cement is an effective local antibiotic delivery system and a biocompatible material for filling the debrided space to facilitat...

Antibiotic-laden Arthroplasty with a Novel Design of Cement Mould and Metallic Endoskeleton for Treatment of Hip Infection, an Inexpensive Alternative: A Preliminary Report of Two Cases

Infection of the hip after implant fixation is an uncommon yet devastating complication that results in poor long-term outcome. The gold standard treatment for chronic infection after hip arthroplasty is a two-stage protocol: eradication of infection, follow by re-implantation arthroplasty. The use of interim antibiotic-laden cement spacer has become a popular procedure to maintain hip joint fu...

Treatment of Proximal Femur Osteomyelitis Occurred after Proximal Femoral Nail Antirotation Fixation, with Antibiotic Cement-coated Tibia Intramedullary Nail: A Case Report

Antibiotic cement-coated intramedullary nails maintain a locally high antibiotic concentration while contributing to bone stability. We present a case of femoral subtrochanteric fracture in a patient with an infected nonunion who was successfully treated for an infection and nonunion using an antibiotic cement-coated tibial intramedullary nail. A 79-year-old woman with a right femoral subtrocha...

Daptomycin: Local Application in Implant-Associated Infection and Complicated Osteomyelitis

BACKGROUND The rise of highly resistant bacteria creates a persistent urge to develop new antimicrobial agents. This paper investigates the application of the lipopeptide antibiotic daptomycin in infections involving the human bone. METHODS Compressive and tensile strength testing of daptomycin-laden PMMA was performed referring to the ISO 5833. The microstructure of the antibiotic-laden PMMA...
